# Prepare a medical-record request follow-up list

Canonical page: https://agents.paretoid.com/workflows/prepare-a-medical-record-request-follow-up-list/

Category: Client Service

Practice relevance: specialty-specific. Medical-record request chasing is a distinct PI-oriented job supported by original PI pain evidence; not an Australian provider integration.

Jurisdiction/provider scope: Clio Manage Personal Injury Medical Records provider cards or approved local request records. Confirm account access and authority; no assumed Australian medical-record integration.

Use request-status metadata to find due follow-ups and stop chasing received records.

Status: Proposed / not tested. The public discussion supports the problem; the method source supports only the documented features. No outcome from this adapted recipe has been verified.

Verification: The linked sources support the complaint and documented method components. We have not run this recipe or verified its results. The example below is synthetic, not a customer result.

## The problem

Use request-status metadata to find due follow-ups and stop chasing received records.

Why it matters: An unanswered records request can delay work, but repeated follow-ups after receipt waste staff and provider time. A status-based review list helps staff focus on requests that still need an authorized check.

## Sources and evidence

- pain: [Pain evidence: How to make requesting medical records more efficient?](https://www.reddit.com/r/paralegal/comments/1scmzpc/how_to_make_requesting_medical_records_more/)
  How to make requesting medical records more efficient?, 2026-04-04, older. A three-person PI firm reports that request chasing consumes its day and delays cases. Multiple comments independently describe repeated follow-ups and provider-routing friction.

- method: [Method: Arctrieval request activity guide](https://arctrieval.com/wp-content/uploads/2025/09/Arctrieval-Request-Activity-Guide-2025_09_24.pdf)
  Arctrieval request activity guide documents enabling/disabling follow-up and request activities. Its existing authorized system is a capability option, not a new data destination recommended by this research.

Source checked: October 15, 2026. Pain reports are self-reported and identities are unverified. Method documentation supports components, not a tested end-to-end workflow.

## Suggested approach

Start when: Approved follow-up date reached

1. reconcile status, last action and response flag

2. Keep row or page references and mark missing or ambiguous evidence. Do not apply the suggested changes.

3. The responsible staff member checks the draft against the source records and approves any separate action.

## What you need

- Clio Manage Personal Injury add-on with maintained provider request cards, or actual approved request letters and response log available locally.

- Administrative follow-up policy and explicit staff-approved follow-up dates; no patient identifiers or clinical facts in the AI tables.

- Firm-approved Excel; optional Copilot entitlement and tenant approval for the exact minimized data. Native Excel analysis can be done manually if Copilot is absent.

- Keep raw exports and identifying source pointers in a separate restricted location. The AI workbook contains only approved alias-based tables and visible policy/scope text, never raw client records on another tab.

- Use synthetic inputs first. Keep real data in firm-approved accounts and processing, with appropriate access, confidentiality and retention controls. Aliases alone do not anonymize records. Never paste credentials, API keys or unnecessary client details.

## How to use

Documentation-checked proposed route; source collection and human approval required

- The Personal Injury add-on is plan-dependent. Its provider cards track request dates/status/follow-up, but this research does not verify a request-status CSV export or an AI query for the kit schema.

- Cards are staff-maintained and may be stale; last_response_date and paused require actual response/pause records. A Received label does not establish medical completeness.

Access and setup:

- Actual account access, plan and source-data completeness have not been tested.

### 1. Read the actual administrative request state

Tool: Clio Manage Personal Injury, or local source records

Location: Matters > selected matter > Medical Records > provider card

Read the request date, recorded request status and follow-up date separately for records and bills. Use the provider card’s existing details if needed without saving changes. Check the latest actual response and any pause instruction locally. If the add-on is absent, read the sent request letter, receipt acknowledgment and maintained follow-up log directly; do not assume another CRM can export them.

Expected output: One verified administrative record per request type, with source evidence. Exclude treatment dates, diagnoses and record contents.

Next step: Assign non-identifying request_id and public_provider_id; keep client/matter lookup restricted.

Check before continuing: Do not treat the card’s default Not yet requested as proof if the actual letter shows otherwise; flag contradictions.

Execution status: Official documentation checked 2026-10-15; account and workflow not personally tested

Step documentation: https://help.clio.com/hc/en-us/articles/15815962459931-Medical-Records

### 2. Map the observed records to the starter input

Tool: Excel and authorized staff

Location: Separate analysis workbook > Home > Format as Table

Create request-status.csv with request_id,public_provider_id,sent_date,last_response_date,status,staff_approved_followup_date,paused. Separate records and bills into different request IDs where they have different histories. Copy followup-policy.txt into Policy; specify the review date/timezone in Scope. Unknown response/pause evidence remains unknown. Create named tables for RequestStatus. Put the policy and review scope in visible worksheets or paste them into the next prompt.

Expected output: Reviewed administrative RequestStatus table plus Policy and Scope.

Next step: Check the mapping against each original source before using AI.

Check before continuing: Aliases and missing policy fields are staff-created; do not relabel a native export as the kit schema. Unknown stays blank/unknown, not false or zero.

Execution status: Official documentation checked 2026-10-15; account and workflow not personally tested

Step documentation: https://support.microsoft.com/en-us/excel/tutorial-import-data-into-excel-and-create-a-data-model

### 3. Review the bounded evidence with Copilot

Tool: Excel Copilot, optional

Location: Approved analysis workbook > Copilot icon at lower right > Chat only

Use Chat only and submit the prompt. Without Copilot: filter actual approved dates/statuses and apply the policy row by row.

Prompt:

```text
Apply Policy to RequestStatus at the supplied review date. Return request_id, public_provider_id, observed status, approved follow-up date, pause status, recommended_review_action and evidence rows. Separate due, not yet due, paused, received, incomplete and unknown. Do not invent dates or use treatment information. Draft a neutral administrative follow-up only for a staff-approved due request with no unresolved pause/status issue. Do not include patient identity, send a message or decide record sufficiency.
```

Expected output: Administrative follow-up queue and safe draft placeholders.

Next step: Copy the draft exception list to the reviewer; retain row references.

Check before continuing: Check every flagged row and the scope. A fluent answer is not evidence of completeness; no automatic workbook or source-system edits.

Execution status: Official documentation checked 2026-10-15; account and workflow not personally tested

Step documentation: https://support.microsoft.com/en-us/excel/copilot/get-started-with-copilot-in-excel https://support.microsoft.com/en-us/excel/copilot/frequently-asked-questions-about-copilot-in-excel

### 4. Approve the review result

Tool: Responsible staff reviewer

Location: Reviewed local worksheet

Records staff verify each candidate against the sent request and latest response, then the responsible lawyer approves any unresolved status. Any provider message is prepared and sent separately through the approved secure channel.

Expected output: Reviewed follow-up list with blocked/unknown requests visible.

Next step: Any external message, source change, payment or deletion requires its own authorized process.

Check before continuing: Never infer no response from a missing log entry, or use an AI draft as authority to request more medical data.

Execution status: Official documentation checked 2026-10-15; account and workflow not personally tested

Step documentation: https://support.microsoft.com/en-us/excel/tutorial-import-data-into-excel-and-create-a-data-model

## What you will get

followup-review.csv: request_id, staff_approved_followup_date, status, evidence_rows, review_reason, placeholder_draft. No live changes.

## Human review and limits

Authorized staff verify recipient and patient authorization inside the existing secure system, then approve any contact. A received request must stop follow-ups.

The assistant drafts or reports only. Check source records and review the output before a human performs any separately approved send or change. No automatic legal decision, money movement, account connection or publication is included.

What to measure: During the pilot, count findings confirmed by the reviewer, false alarms, and missing-input cases. Do not infer time or money saved.

## Original prepared-input schema

request-status.csv: request_id, public_provider_id, sent_date, last_response_date, status, staff_approved_followup_date, paused. No patient identity, patient-level dates or health content.

Synthetic example input:

```text
REQ1: received, follow-up due yesterday
REQ2: pending, follow-up due today
REQ3: pending but manually paused
```

Expected synthetic output:

```text
REQ1: exclude
REQ2: staff review with placeholder draft
REQ3: exclude while paused
```

Failure check: Authorized staff verify recipient and patient authorization inside the existing secure system, then approve any contact. A received request must stop follow-ups.

## Optional original prepared-input prompt

Destination: Your firm-approved AI assistant · prepared files

This original prompt uses its own prepared-input schema. Do not mix it with native-step tables without the supplied mapping.

```text
From this deidentified request-status table and staff-approved follow-up policy, list requests due for staff review. Skip received, cancelled and paused requests. Preserve request IDs and dates; flag conflicting status. Produce generic draft text with placeholders only. Do not infer statutory timing, threaten complaints, contact providers or request records.
INPUTS
request-status.csv: request_id, public_provider_id, sent_date, last_response_date, status, staff_approved_followup_date, paused. No patient identity, patient-level dates or health content.
OUTPUT
followup-review.csv: request_id, staff_approved_followup_date, status, evidence_rows, review_reason, placeholder_draft
Use only the supplied synthetic or firm-approved inputs. Cite source row or page IDs for each finding. Keep unknowns unresolved. Return a draft for the responsible staff reviewer. Do not send messages, connect accounts, change records, move money, or make legal decisions. Keep source files unchanged. If an input or permission is missing, list what is needed and stop that check.
